Funeral services for Johnny Dale Smith, of Monticello, were 2 p.m. on Sunday, Oct. 7, at Calvary United Pentecostal Church in Jayess, with burial in Powells Grove United Pentecostal Church Cemetery. Riverwood Family Funeral Service is in charge of arrangements.

     Mr. Smith, 50, died Oct. 4, 2012, at The University of Mississippi Medical Center in Jackson. He was born in Mississippi on March 31, 1962, to Johnny Bert and Betty Lou Smith “Smith”.

     He was a truck driver and of the Pentecostal faith.

     Preceding him in death were his parents.

     Survivors are his wife, Rachel Womack Smith; son, Joshua Dale Smith and wife, Angela Marie; daughter, Saranda Ann Smith; brothers, Danny Smith and wife, Peggy, and Mitchell Smith; stepsister, Sue Fairley; three grandsons; and three granddaughters.
